Inpatient/Outpatient Surgical Benefit*
If you are under age 65, it pays benefits for medically necessary treatment of an injury or sickness according to the surgical schedule and the description of coverage provided in your membership materials. The maximum amount payable for all surgical procedures performed in a calendar year for a covered person is $20,000.

Individual of Family - from $40 to $4,000 per covered surgery according to the surgical schedule
Signature 1000
Individual of Family - from $80 to $8,000 per covered surgery according to the surgical schedule

Coverage is not provided for members over the age of 64. Coverage becomes effective on the date provided in your membership materials.
