Doctor Visits
The Doctor Visit Benefit is under the Group Accidental Death and Dismemberment and Medical Care Insurance policy and is an added benefit of your membership.
The Doctor Visit Benefit pays you the amount listed per visit to a doctor's office to be diagnosed or treated for a covered accident or sickness. You will be paid for up to 5 visits per person or 5 visits per family per year.
Signature 300
Individual - $25 per visit up to 5 visits /year per person
Family - $25 per visit up to 5 visits /year per family
Signature 1000
Individual - $50 per visit up to 5 visits /year per person
Family - $50 per visit up to 5 visits /year per family

All members of the HealthCard Now Signature 1000 and Signature 300 programs are enrolled into America's Health Care Consumer Association (AHCCA) to be eligible to receive these benefits. United States Life is domiciled in the state of New York with a principal place of business of 70 Pine Street, NY, NY 10270 and licensed in all states, plus DC, except PR. These benefits are under Group AD&D & Medical Care Insurance Policy (Form #G19000) issued to America's Health Care Consumer Association (AHCCA) in Illinois. Coverage is not provided for members over the age of 64. Coverage becomes effective on the date provided in your membership materials.
